Practice Issue Statement, Scope of Practice, Practice Area

Identifies a practice issue statement that is an independent nursing decision, and is relevant to the systematic review topic chosen from among the approved list provided in the course. Describes the scope of practice issues. Example: The number of falls in your facility has been increasing, especially during shift change. What do you believe may be causing this? Are there any new interventions that could be put into place that would lessen or eliminate the falls? This is where you begin your initial search into the problem or practice issue identified. Identifies the practice area.

Practice Issue Worksheet

List the topic and include the citation for the systematic review you have selected from our approved list (optional: an additional scholarly source of support):

Topic: Health-Associated Infection

Citation: Cooper FPM, Alexander CE, Sinha S, Omar MI. Policies for replacing long-term indwelling urinary catheters in adults. Cochrane Database of Systematic Reviews 2016, Issue 7, Art. No.: CD011115. doi:10.1002/14651858.CD011115.pub2

What is the nursing practice issue you have identified related to the topic you have chosen?

The practice issue is to identify the policies for handling indwelling urinary catheters in adults that would reduce the risk of infection. It recognizes that there are different policies for handling catheters, but each policy has different strengths and weaknesses. As such, analyzing the merits and demerits of each policy would help in identifying the best policy for reducing the risk of infection for the patients and ensuring all round safety for the medical personnel.

Fully describe the scope of the practice issue:

Catheter management is influenced by a range of factors, key of which is cost, convenience and risk. The catheters cost adds to the cost of care while convenience is concerned with how medical personnel plan their professional responsibilities. Risk includes its association with both the patients and medical personnel. Catheters should ideally be replaced when the old ones are no longer useful and the replacement should not place the medical personnel at risk. As such, any policy for managing catheter replacement should include considerations for cost and convenience, as well as risk to patients and medical personnel. In identifying a policy that combines the three factors, this research anticipates to present a viable catheter management policy that meets the mentioned needs to cost, convenience and risk.

What is the practice area?

_X_ Clinical

___ Education

___ Administration

___ Other (List):

How was the practice issue identified? (check all that apply)

_X_ Safety/risk management concerns

_X_ Unsatisfactory patient outcomes

___ Wide variations in practice

___ Significant financial concerns

___ Difference between hospital and community practice

_X_ Clinical practice issue is a concern

___ Procedure or process is a time waster

___ Clinical practice issue has no scientific base

__ Other:

Describe the rationale for your checked selections:

Incorrect handling of catheters is likely to place the medical personnel at risk. Additionally, erroneous management of catheters increase the risk of the patients acquiring new urinary tract infections. As such, the research intends to reduce the possibility of unsatisfactory patient outcomes by improving the patients’ health outcomes.

Also, it intends to improve clinical practice by identifying the most efficiency way for handling catheters.

What evidence must be gathered? (check all that apply)
_X_ Literature search

_X_ Guidelines

___ Expert Opinion

___ Patient Preferences

___ Clinical Expertise

_X_ Financial Analysis

_X_ Standards (Regulatory, professional, community)

_X_ Other – Medical records

Describe the rationale for your checked selections:

The literature search will explore publications that are relevant to the research topic.

The guidelines will be reviewed to see policies that govern catheter management.

Financial analysis will be conducted to review the costs of each policy option, particularly the cost of catheters and their use.

Standards will be identified to have a better awareness of the existing thresholds.

Medical records will be reviewed to evaluate the effect of each policy on urinary tract infection incidence.

Evidence Summary Worksheet

Directions: Please type your answers directly into the worksheet.

Describe the practice problem in your own words with reference to the identified population, setting and magnitude of the problem in measurable terms:

The practice problem is that incorrect catheter management increases the risk of urinary tract infection. This is particularly true for adult patients since the large majority of them are comprised of elderly persons identified as high risk populations.

Type the complete APA reference for the systematic review article you chose from the list provided. It must be relevant to the practice issue you described above. Include the APA reference for any additional optional supplemental scholarly source related to the review you wish to use.

Cooper, F., Alexander, C., Sinha, S. & Omar, M. (2016). Policies for replacing long-term indwelling urinary catheters in adults. Cochrane Database of Systematic Reviews, 2016(7), Art. No.: CD011115. doi:10.1002/14651858.CD011115.pub2

Identify the objectives of the article:

The article’s objective was to evaluate the different policies for replacing long-term indwelling urinary catheters in adults with the focus being on their effectiveness.

Provide a statement of the questions being addressed in the work and how these relate to your practice issue:

The question asked in the research is: What are the policies for replacing long-term indwelling urinary catheters in adults and how effective are these policies?

This question relates to the current practice issue by providing evidence to support emphasis on specific policies. That is to say that it identifies the more effective policies, showing reason for their advantages.

Summarize (in your own words) the interventions the author(s) suggest to improve patient outcomes.

The article’s summary notes that the set objective was not met. Still, it made some good contributions towards understanding on the policies. In fact, it shows that an effective catheter management policy policies must consider the patients’ satisfaction, quality of life, convenience, type of trauma on the urinary tract, and economics. Additionally, the authors raise the question of whether or not catheters can be replaced.

Summarize the main findings by the authors of your systematic review including the strength of evidence for each main outcome.  Consider the relevance to your project proposal for the Milestone 2 project paper. (If an optional supplemental source is also used, include a statement of relevance to it as well.)

The article summarizes that it does not have sufficient evidence to reach a conclusion on the effectiveness of the difference catheter replacement policy.

Outline evidence-based solutions that you will consider for your project.

The present project will consider the risk associated with each policy, particularly the infection incidence rate associated with each policy.

Discuss any limitations to the studies that you believe impacts your ability to utilize the research in your project.

The major limitation is that there are a lot of variables that could affect the research results. Some of these variables include differences in catheter brands, lubrication used, environment (whether in hospital or at home), and the catheter replacement strategy.
